RE:                   Matthew N. Burgess – Stipulation of License Surrender

DATE:             September 20, 2000

 

On or about the ninth day of September 1999, Mr. Matthew N. Burgess resigned his teaching position at Terre Haute North High School.  The Superintendent of Public Instruction obtained a Stipulation of License Surrender from Mr. Burgess who holds Indiana Teacher’s Licenses #662845 and #642552 that expires on September 11, 2002 and #644698 that expired on June 30, 1998.

The revocation file is available for inspection upon request

 Recommendation:

 The staff recommends that the Board accept the Stipulation of License Surrender for Matthew N. Burgess for Indiana Teacher’s Licenses #662845, #642552 and #644698 thereby revoking his licenses.
