NOTE: To determine your eligibility for an Indiana license, gather the materials below and send them to the Indiana Professional Standards Board at the address listed below. Check the items which are being sent and return this checklist with your materials. Failure to do so will result in delays in processing your evaluation request.

Evaluation Fee Each level of evaluation requires a $35.00 cashier's check or money order. Click on evaluation to review the various licensing levels. DO NOT SEND CASH OR A PERSONAL CHECK. 
Foreign Credentials Evaluation Report Enclosed You must obtain an official evaluation report which translates your non-U.S. course work into U.S. equivalents. Contact the Educational Credential Evaluators or World Education Services to obtain this report. NOTE: The Indiana Professional Standards Board can not evaluate your credentials without this report. 
Verification of U.S. Equivalent Student Teaching When you obtain the foreign credentials evaluation report, it must include student teaching as a part of your preparation program (if you are requesting a teaching license). NOTE: If your report does NOT include student teaching, the Indiana Professional Standards Board will not evaluate your credentials, you must enroll at an Indiana college or university to obtain your Indiana teaching license. 
Valid Out-of-State License Send photocopies of both sides of your unexpired out-of-state license. NOTE: Only licenses issued in U.S. jurisdictions may be accepted. 
Teaching Experience Dates of employment, positions held, grades and subjects taught or administered, whether the experience was full or part time, and accreditation of the school must be verified on official school letterhead from the employing superintendent or principal. The last school corporation that employed you may verify all of your experience. NOTE: Only experience gained in U.S. accredited schools may be accepted. 
State Form 9331 Complete the application for Indiana Teaching, Administration, or School Services License. 
Official NTE Score Reports Official score reports of the NTE Core Battery, Pre-Professional Skills Test (PPST) and/or Specialty Area tests should be sent directly from the Educational Testing Service (ETS). Contact ETS at 800-772-9476.
Limited Criminal History Report Attach the original limited criminal history report after the Indiana State Police sends the report to you.(Effective July 1, 1997, all applicants for licensure must submit limited criminal history reports with their teacher license applications. The report is valid for one year.)
