PDP Representatives

For the initial PDP, the teacher should use the PDP representative teaching the 45-clock hour seminar. When renewing a Workplace Specialist Proficient Practitioner License, the teacher should contact the PDP representative listed on this page. Please note that one turns in a PDP with a signature of a PDP representative other than one listed on this page, then considerable delays in renewal will occur and the PDP will be returned for the appropriate signatures.
