In the sections Record Keeping Forms for Centers and Record Keeping Forms for Family Day Care Homes on the CACFP web page, you will notice we have added a Multi-Year CACFP Enrollment Form.
Institutions DO NOT have to use both forms. The CACFP Enrollment Form is good for one year. When it is time to renew the information, the parent must complete and sign a new form.
The Multi-Year CACFP Enrollment Form can be renewed by just having the parent resign the form. This form can only be used if there are NO CHANGES to the original submission.
CACFP participants are unique. Choose the one that works best for your facility.
