Recipient Agency Delivery Location Sheet Instructions

The Recipient Agency Delivery Location Sheet provides contact information needed to have commodities delivered to a specific drop site. One of these forms needs to be completed for every location where commodity deliveries are made. This form must be completed prior to placing commodity delivery orders. Throughout the school year whenever any information changes, it is the responsibility of the Recipient Agency to revise and resubmit the location sheet.

Delivery Address (lines 1-5) – This is the physical location where commodities are actually delivered. This address could be a school, central kitchen, or other facility that is to receive commodities. Remember, this form must be completed for every drop site. DO NOT USE A P.O. BOX.

Delivery Contact (lines 6-12) – This is the business information for whoever is designated as the primary contact at each delivery location. It is important that this information is accurate because this person will be the first contacted whenever important delivery information needs to be communicated. Please
double check the e-mail address to assure accuracy.

Alternate Delivery Contact (lines 13-20) – This is the business information for whoever is designated as the Alternate Delivery Contact. It is important that this information is accurate because this person will serve as the back–up to the Delivery Contact designee. This person will be contacted whenever the Delivery Contact cannot be reached. Please double check the e-mail address to assure accuracy

Delivery Information (line 21) – This is the information needed regarding the storage capabilities for each delivery site. Please check all that may apply. Orders for commodities delivered to a site that does not have the appropriate storage capabilities may be requested, however a warning window may appear.

NOTE: It is important to check this page annually for accuracy. You should update the information and make the appropriate changes as they occur throughout the year. Additionally, you may add a delivery site at any time but it is necessary that you contact the state agency before attempting the process. You must click the “submit” button at the bottom of the page anytime changes are made to the information. Commodity delivery orders may not be created for any site that does not have a “Submitted” Delivery Location Sheet completed. If you have submitted the document it will have a status of “Pending Approval”. If you have not submitted the document, it will still have a status of “Pending Submission”.
