Instructions for Online Sponsor and Site Application

Go to our website, click on Production Site button.

Enter login and password.
3.
Click on SFSPC puzzle piece.
4.
Click on 2007.

The Sponsor Summary screen should come up. Click on the blue applications tab under the Sponsor Summary title.

The Sponsor Info sheet, Sponsor Budget, and Site Info sheets and should now be listed.

Click Add to the right of the Sponsor Info sheet. Complete Sponsor Info sheet and hit submit at the bottom. You will have to correct any errors that come up and then the Sponsor Info sheet should be Pending Submission.

Non-school sponsors and new school sponsors will then have to hit add for the Sponsor Budget and complete and submit it. School sponsors not new to the SFSPC are not required to submit a budget.

Then you need to click add for each site wish to operate. If you need to add or delete a site you will need to call into the office. All sites must be completed before you can submit your packet.

Once your packet is complete you must submit it to the State Agency. Click on the packet tab under the Sponsor Summary tab and check the box to submit to the State Agency. Your packet will then be Pending Approval until the State Agency approves it.

If you add a site after you submit your packet you will submit that site individually, there will then be a box at the bottom of the site application to submit to the State Agency.
