How to Convert your Initial Practitioner License to a Proficient Practitioner License

Administrator Licenses

Administrators will submit a copy of their CPR card, ISLSA results, the Individual Development Plan Summary, complete a profile and an application, and pay for the application through LVIS.

Once your account is set up, go to “add application” on the right hand side of your LVIS account. A new screen will open up, pick "Conversion" from the application action. Wait a few seconds while the system finds your license to convert. Then select "Conversion Administrative-Initial Practitioner license".

You will be prompted to download your Moodle report card and Individual development Plan Summary.

Submit and pay for your application through LVIS and then the IDOE will take care of everything else.

Instructional and School Services Licenses

Once your account is set up, go to “add application” on the right hand side of your LVIS account. A new screen will open up, pick conversion from the application action. Wait a few seconds while the system finds your license to convert. Then select conversion instructional-initial practitioner license.

You will be prompted to download your IMAP completion letter and your CPR/AED/Heimlich certification. Effective July 1, 2012, applicants for initial licensure of any type and applicants for all renewals must show proof of valid CPR/AED/Heimlich certification. How to Convert your Reciprocal Permit to an Educator License

This process is available to those who have had their credentials evaluated by the Office of Educator Licensing and Development and received an Evaluation/Deficiency Letter (may have been emailed to you through LVIS) stating that deficiencies need to be fulfilled to establish an Indiana Educator License. To apply:

  • In LVIS in the application section, look for “Licenses: Reciprocal Permit Conversion to Standard License.” You will need to be sure to scan your official documents showing completion of your deficiencies.
  • When completing your application in our online License Verification and Information System (LVIS), attach a copy of your Evaluation/Deficiency Letter (may have been emailed to you through LVIS), Proof of Completed Deficiencies, your CPR/AED/Heimlich training card, and your Suicide Prevention training certificate (click here for training information).
