Student Safety/Bullying Reporting
Indiana's Student Safety Reporting law (IC 20-34-6-1) requires school corporations to report student bullying and arrest data for each school to the Indiana Department of Education (IDOE) by July 1 of each year.
Arrest Report Form
The Student Safety Reporting Form is a template that schools and school corporations can use to submit the student arrest data to the IDOE. The Student Safety Reporting Form Sample can be used if you have questions as to what data the IDOE need to collect or if you have any questions about the form. Schools and school corporations may also choose to use their own forms to submit the data. All forms should be submitted to IDOE at firstname.lastname@example.org. For purposes of this report, an arrest is when a child is physically taken into custody by law enforcement while at school, or as a result of an action taken at school.
Bullying Report Form
To submit your final bullying incident report form to the IDOE please use the following link. This link will take you to a JotForm survey where you can submit your number of bullying incidents by category.
We have created a Bullying incident report Form that can be used to track your bullying incidents over the course of the school year. Please report all incidents only after the investigating school official determines that “bullying” has actually occurred under the expanded 2013 definition at IC 20-33-8-0.2.
The bullying reporting form sample can be used if you have questions as to what data the IDOE need to collect or if you have any questions about the form. Please note, incidents that qualify as "Combination" incidents should only be reported once, rather than reporting in multiple fields.
