Custodial Agreement Forms

Spanish

As families try to re-establish their living situations, they may leave children in the care of relatives or friends here in Indiana while they travel to their home states. In such instances it would be wise to complete a third party custodial form to ensure that an adult has clear authority to make educational decisions if needed.
