Do we report students with legal residence in our district for whom we provide transportation
that attend the deaf or blind school?

The school for the deaf or blind will report these data since they provide the special education
service to the student. They will use your corporation as the corporation of legal settlement. |

We have a student we are home bounding. Does he count on the SE?

This student will be submitted if he is a special education student with homebound placement.
Check with your special education director on the exact meaning of the homebound placement. |

We have some PK students who are attending private schools that receive special needs services
from our corporation, but are never physically in our school buildings. I need to know how to fill
in Fields 1 (School Number) and 9 (Facility) for those students.

PK students are included if they have IEPs and your corporation provides the special needs
services. Enter the school number of the private school in Field 1 if it has a state-assigned school
number. (Use the "Corp and School Lookup" in the Application Center to find this.) Field 9
(Facility) could be "01," "05," or "07." Check with your Special Education Director for details
about the private school. If the private school is a facility and does not have a state assigned
school number, use "9999" in Field 1 (School Number) and use Field 9 (Facility) to identify the
type of facility. |

I have 3 PK speech students who reside in our district and attend speech in a Co-op in another
corporation. I have asked the other corporation if they are reporting them on their SE report and
they said no, it was our responsibility. If I upload them, what do I put as the school number and
facility code?

Since the students are receiving services at a school outside of your corporation you will not be
reporting the students. The school that is providing services is the school that reports the
students. |

A student tested out of special ed but we are still supplying him with some services. Do we
continue to count him on the SE report?

A |
A student does not "test out of special education." If a student, as determined by a legally
constituted case conference committee, no longer requires special education these services would stop and the student would not be counted on the SE. In order to be counted on the SE, a student
must be receiving special education services under an IEP as determined and developed by a
case conference committee. If a student receives "some services," but not as specified in an IEP,
the student would not be counted on the SE. |

When coding a preschool student at Head Start, what facility type code would you use?

Facility, Field 9 would be "02." |

We are serving students in an accredited private school. Would the Facility code (Field 9)
be coded as: 01 or 99?

Because the accredited private school has a state-assigned school number, that number is
used in Field 1 (School Number). Enter "99" in Field 9 (Facility) and "000" in Field 10
(Service Site) because Field 1 (School Number) has a state-assigned number. |

We are providing services for students at a neglected and delinquent institution in our
district. I found the school number on the neglected institutions reference list. Do I use
that number in the Service Site field?

The reference list for Neglected Institutions should be used for reporting the DOE-TI
Title I. The DOE-SE Special Education should use the SERVICE SITE PROVIDER List
(http://www.doe.in.gov/stn/pdf/se_providers.pdf). |

We have a special needs child who is currently in 6th grade. Because of her special
needs, she has been placed in a severe and profound facilities located in the elementary
school building. Normally, the elementary school only educates up to the 5th grade, so
we are getting an error.

For this student we will make a grade exception to your school when it is a special
education student. You can contact our office and you will be directed to the appropriate
staff. |

We have several students who live in our district and receive special education services but
attend a private school that is not accredited and does not issue STNs. Will I have to assign an
STN to these students and input them prior to the special education count in order to avoid the
error, "The STN does not exist in the STN Lookup System."

Yes, if they do not already have STNs in the Application Center, you will need to create them.
Use your school's number as the first four digits since you are the only accredited school
providing services to this student. |

How do I delete students from the SE report once they have been reported?

If SE records are in conflict, you can delete them on the Conflict Resolution page (Message
Center > Conflict Resolution). Any SE records (in conflict or not) can also be deleted by clicking
on the red X next to the record below the Input Form (Data Transfer > Input Forms > Special
Education Input). |
