Some of the questions I have been receiving concerns the categories of Asthma. The three classifications of mild, moderate and severe comes from, most recently,the EXPERT PANEL REPORT, GUIDELINES AND MANAGEMENT OF ASTHMA, USDHHS, 2007.    Mild asthma is characterized by “absence of any sign or symptom,no inhaler use” If you have an inhaler ordered and you have not used it, even though you have a diagnosis of ASTHMA from the parent, this would be mild asthma.   Moderate asthma is characterized by” inhaler use > 2 times a week with night symptoms” ( I know if they self carry, or unless, you have contact with them, you may not know about the night symptoms).   Severe asthma is characterized by “ symptoms, inability to perform normal activities < 2 days a week, absenteeism, 2 or more ED visits”. 

EH or Emotional Disability are not counted as a chronic condition unless they have one of the diseases  on the list as a secondary condition, because they will have been identified as Special Education and will be counted in that count.  Your students counted in OHI, are in that category, but may have chronic conditions listed in the categories for chronic diseases.

Does this include all grades? Preschool and Kindergarten also?

  A Every student enrolled in your school corporation, including homebound  that has a chronic condition as referenced in the Memorandum.

Please define what the state wants reported as an allergy.  We have a number of students whose parents report an allergy to mosquitoes or bees because the site swells if they are bitten or stung or a strawberry allergy because it upsets their stomach.  Are you wanting us to report all parental reports of allergies to foods and insects no matter how slight the symptoms, or only those allergies that result in an anaphylactic reaction?

If you have used the information from the parents for your own purpose, please use it again for this report.  There are three categories, as you can see, list the anaphylaxis under those categories. Add , if you desire, the anaphylaxis  to the other category.
