• The First window to submit data will be August 1, 2006 – August 28, 2006.
• Barcode labels delivered September 8, 2006.
• The Second Window to submit data will be August 29, 2006 – September 7, 2006. This window is for NEW DATA ONLY.  Submit data that was not submitted in Window 1.
• Barcode labels submitted during the second window will be delivered September 15, 2006
• The Third Window to submit corrections to data submitted during Windows 1 & 2 will be September 8 through September 27, 2006.
