We have a young lady who is a special needs child.  She is currently a 6th grade student.  Because of her special needs, she has been placed in the elementary school, severe and profound facilities are only located in the elementary school building.  The state, that would be you, lists South Newton Elementary as a school that only educates up to the 5th grade.  So, therefore, there is a conflict with the SE data.  The recommendation is to contact an administrator to discuss this conflict.  So, who would that be?  Please help.

For this student we will make a grade exception to your school when it is a special education student. You can contact our office and you will be directed to the appropriate staff.

Do we report students with legal residence in our district that attend
the deaf or blind school, for whom we provide transportation?

  A

The school for the deaf or blind will report this data since they provide the special education service to the student. They will use your corporation as the corporation of legal settlement.

I have two questions, 1st a student is expelled as of September and allowed to come back in March, but we are home bounding him.  Does he count on the SE?

  A

This student will be submitted if he is a special education student with homebound placement. Check with your special education director on the exact meaning of the homebound placement.

When coding a preschool student at Head Start, what facility type code would you use?

  A

Facility, field 9 would be 02.

We are servicing students in an “accredited private school” would the facility code be coded as: 01 or 99?

  A

Because the accredited private school has a state assigned school number (4 digit number starts with A-E), the number is used in field #1, 99 in field #9 because the state assigned number is provided and 000 in field #10 because field #1 has the state assigned number.

We are providing services for students at a neglected and delinquent institution in our district. I found school number on the neglected institutions reference list. Do I use that number in the service site?

The reference list for Neglected Institutions should be used for reporting the DOE-TI Title I. The DOE-SE Special Education should use the SERVICE SITE PROVIDER List.

For the CODA report they are now asking for facility codes. Are you familiar with these? They are on the DOESE Report (Field 1) We are not sure what to use for Head Start and for our walk-in speech center. Can you give some help or send us to the person that can help us?

Mark "district wide preschool" for all preschool children with IEPs regardless of where they are served.

Please clarify if we are to include PK students or any other student that we provide Special Services for who are not attending one of our schools. We have some students who are attending private schools, that still receive special needs service from our corporation, but are never physically in our school buildings. I need to know how to fill in #1 and #21 for those students.

  A

First check with your Special Education director about what data is submitted by CODA. PK students are included if you provide the special needs service. Use the school number of the private school if they have a state assigned school number. Use the Corp and School Lookup in the Application Center. Field 21 could be 01, 05 or 07. Check with your Special Education Director for details about the private school. If the private school is a facility and does not have a state assigned school number use the 9999 and use field 21 to describe the facility.
