Chronic Disease Report (DOE-CH) for the 2007-08 School Year |
Public Law 166-2007, Section 8 requires school corporations to report the number of students in the school corporation who have a chronic disease, by disease category. Please share this memo with your school nurses.
The number of students who have a chronic disease will be reported on DOE-CH Chronic Disease. This is not an STN Application Center collection. This corporation level report will be collected electronically on the DOE-Online, with corporation number and PIN access required. It is a count of students by category. A student may be counted in multiple categories but may be counted only once in the unduplicated column.
The law defines “chronic disease” as asthma, diabetes, and any other disease the Department of Education determines is significant for the school corporation to report. In determining the list of diseases to be reported, the Department used a descriptive definition of “any physical or mental condition that requires long-term monitoring and/or management to control symptoms and to shape the course of the disease.” The Department reviewed information from the United States Department of Health and Human Services, Centers for Disease Control and Prevention, Pediatrics texts, and a previous count taken in 1998 by the Department’s Division of Special Education. School nurses are familiar with the listed conditions.
The count will be taken on December 3, 2007. Although the law likely contemplated a September 14 collection date, December 3 has advantages. Principal among these is that school nurses will have better information about conditions affecting students. We explained our rationale for using a December collection to the General Assembly’s Health Finance Commission, and the members accepted the decision.
The deadline for submittal of this information is January 11, 2008.
