Technology Plan Update

The Technology Plan Update form for 2008 should be completed by each school corporation and should include updates on how technology is being used in your corporation.  If your corporation uses a PBX for voice services, you should include a description of that technology as well and how it improves education.  You may use as much space as needed to describe your updated technology plans.

2008 Service Level Checklist

The 2008 Service level Checklist is an important document that must be returned before the Department files for E-rate discounts.  This form allows us to make the proper filing for all school corporations.  This informs the Department about whether you wish to maintain your current service level or increase that level for the coming year.  As we file for discounts, this allows us to be accurate and not to over or under-request discounts for Indiana schools.

You must sign and return the Letter of Agency to authorize the Department of Education can file on your behalf for E-rate discounts.
